School athletes from grades seven to 12 will receive free physicals at the OhioHealth MedCentral Wellness Complex on July 24 at 4:30 p.m. Participants are asked to use the side entrance by Express Imaging located in the complex at 1750 West Fourth St.
The physicals will be first come-first served, but parents must register by calling 419-526-8567. You may call this number for questions or concerns.
A parent or guardian must accompany the athlete and the medical history portion of the form must be filled out prior to arrival. This form may be downloaded from the OHSAA website or obtained at your local school.
