UPPER SANDUSKY — United Church Homes’ Fairhaven Community in Upper Sandusky, Ohio and the local Home Health agency are continuing a long tradition of partnership to ensure continuity of care for those who choose Stepping Stones Fast Track Rehab at Fairhaven Community for their physical, occupational and speech therapy services.

The former organization known as Wyandot County Home Health now is run by Hospice of Wyandot County and is a nonprofit organization called Home Health of Wyandot County. Previously, the organization was part of Wyandot County Public Health, which includes the Wyandot County Health Department.

Fairhaven Community and Home Health of Wyandot County will continue a more than 50-year partnership with the change in leadership. The two organizations have enhanced this partnership through a contract that gives Fairhaven therapists the ability to follow residents home under the umbrella of Home Health of Wyandot County.

In essence, Stepping Stones Fast Track Rehab residents will experience a seamless transition from inpatient therapy at Fairhaven to home health therapy. The partnership places an emphasis on continuity of care and results in positive outcomes for patients.

Fairhaven Community takes great pride in partnering with organizations like Home Health of Wyandot County to continue its mission to transform Aging by building a culture of community, wholeness and peace, Marketing Director Ann Osborn said.

“The folks who come and stay and build relationships with our therapy team here get to be served under the umbrella of Home Health of Wyandot County and build relationships with their nursing team and those same therapists can follow them home,” Osborn said.

Hospice of Wyandot County Director Cathy Browne said she was pleased to continue the partnership.

“We are looking to raise the bar into a new and wonderful partnership,” Browne said. “This is taking care of our community, and Fairhaven has been such a huge part of it for so many years that this is just a great continuum and a great service to our patients and families in Wyandot County.”
