The Crawford County Health Partners met Thursday afternoon to discuss the Community Health Assessment and brainstorm the next step of the process.
In the current funding update for the Community Health Assessment, and Community Health Improvement Plan the CCHP has $42,500. Avita is contributing $12,500, the Crawford County Public Health is contributing $10,000, the Galion City Health Department is contributing $10,000, the ADAMH Board is contributing $7,000, and the Crawford County Partnership for Education and Economic Development $3,000.
During the meeting Nathan Benich explained that the target is to reach $50,000 with some of the money to be used as a sinking fund for the next CHA/CHIP which will be implemented in three years.
The CCHP discussed using AVITA as the fiscal agent for the CHA/CHIP. Avita representative Cinda Kropka explained that she has discussed the matter with Avita CEO Jerry Morasko who said that an agreement would be needed and that no fees would be charged for the health group to act as its fiscal agent.
The CCHP agreed to begin setting up an MOU for future contracts which will begin on Friday.
The CCHP discussed using Sandusky County as an example of how they can utilize the information collected from the CHA.
The CHA questionnaire was also discussed, with the CCHP deciding to ask for a sample question set in order to build the full questionnaire for the CHA.
Thursday’s meeting was held at the Crawford County Partnership for Education and Economic Development offices. The next meeting has been scheduled for 2:30 p.m., April 16, in Galion.
