The Ohio District 5 Area Agency on Aging, Inc. is seeking participation from individuals for a survey to better understand the needs of anyone age 55 and older. If you live in Ashland, Crawford, Huron, Knox, Marion, Morrow, Richland, Seneca and Wyandot counties, your input is needed on a variety of topics including: housing, care and assistance, transportation, health and health insurance, employment and finances, barriers to service and Area Agency on Aging programming.
Results from this survey will be part of a needs assessment to improve programs and services provided by the Area Agency on Aging. The survey will take approximately 15-20 minutes to complete and can be completed at the Area Agency on Aging office at Hawkins Corner 2131 Park Ave. West, Ontario or on line. To complete the on line survey, go to the website www.aaa5ohio.org and click on the Needs Assessment button at the top right corner of the home page.
