CHILLICOTHE — A man originally of Galion died as the result of a shooting Monday afternoon.

According to information released by Chillicothe Police Capt. Ron Meyers, the Chillicothe police and fire departments responded to a report of a shooting in the 300 block of McKeller Street at 4:16 p.m.

In the 911 call, 24-year-old Tyler Hiatt is heard telling the dispatcher he accidentally shot his girlfriend, 23-year-old Tara Carver, in the head. He said he was showing her how to use what he described as an “AR” when the firearm discharged.

During the call, Hiatt said Carver was in the bathroom and pleaded for help to come to the residence. He also said a child was at the residence at the time of the incident.

During the consistent requests for help, Hiatt told the dispatcher he was going to shoot himself and eventually ended the call.

Upon arrival, officers reportedly found Hiatt deceased from an apparent self-inflicted gunshot wound.

Carver was taken by ambulance to Adena Regional Medical Center and then flown to Grant Medical Center by MedFlight, where she later succumbed to her injuries.

The incident remains under investigation.
